Request a sick note

For the first seven days off work, you can self-certify. A sickness certificate from the doctor is not required. A self-certification can be requested from reception. 
A continuation sickness certificate required for in excess of 7 days can be made by submitting a written request to the doctor. This can be done via reception or via email to Admin.NorthCornelly@wales.nhs.uk or by completing the form below. Please note requests can only be made on the day that the sickness certificate is required. We are unable to process requests before a previous sickness certificate has expired.
 
 
Please state:
  • the reason(s) why the continuation is required 
  • the period for which the paper is required
 
A doctor may request to see you or speak with you before issuing a continuation sickness certificate.
