Register with the practice

The Surgeries
 
Patients normally register at the surgery closest to where they live and this is where your medical records will be kept.  However, as all consultation information is recorded on our internal computer system, which is linked to both surgeries, you may request an appointment at either surgery.
 
Area Covered by the Practice
 
The area covered by the practice includes North Cornelly, South Cornelly, Pyle, Kenfig Hill, Cefn Cribwr and Porthcawl* (*only those patients who have moved to Porthcawl having lived in either Cefn Cribwr, Kenfig Hill, Pyle or North Cornelly can remain a patient of our practice).
 
How to Register
  
To register with our practice a registration form (GMS1) and a new patient questionnaire will need to be completed. These forms can be obtained from either North Cornelly/Heol Fach surgery or Stormybrook surgery. A form needs to be completed for each person wishing to register with the practice.
 
The registration form must be signed and dated and contain the following information:
  • Title
  • Name
  • Town & Country of Birth
  • Date of birth
  • Gender
  • Current Address
  • Previous Address
  • Previous Doctor
  • NHS number
Adult patients (over 15 years of age) also need to complete a New Patient Questionnaire.
 
All new patients are required to provide proof of identification upon registration. We require documentary evidence to show where a patient is living (proof of home address) and who a patient is (photo identification).
