Make a Freedom of Information Request

Under the Freedom of Information Act you have the right to access the 'recorded' information held by the Practice, subject to some exemptions.  You can use your rights under this Act to ask for a range of information such as the Practice's policy on data protection or you may wish to know about a particular area of the Practice's business such as planned property development works.
 
Ordinarily there will be no charge for responding to a request for information unless the supply of information requires the Practice to carry out significant amounts of work or photocopying.  Work that takes more than 18 staff hours is regarded as over 'the appropriate limit' (as defined by the Act).
 
You also have the right to request information held by the Practice relating to or affecting the environment (subject to certain exceptions). This could relate to land development, pollution levels, energy production and waste management. 
 
To make a Freedom of Information Act request or to request information held by the Practice relating to the environment, please complete a Freedom of Information Request Form and send this to the Practice Manager at the following address:
